Please include the following in your IRB Review request.
- Application for Use of Human Subjects in Research
- All documents that have been created and will be used in recruitment of participants.
- Informed Consent Form that will be used, hard copy or online precursor to survey with detailed procedure of use.
Include the survey or questions being asked to obtain research data.
- Data collection document as hard copy or printout of online content.
- IRB Request for Exempted Review or Request for Expedited Review.
Exempted Reviews with no follow-up needed should be returned within 2 days
Expedited Reviews with no follow-up needed could take 5-7 days
Full Reviews with no follow-up needed could take 10-14 days
